- Before applying to Online Applicants have valid email & Scanned copies of photo, signature.
- Candidates log on to http://www.nrhmassam.in/.
- Go to "Online Application" & Select the desired post.
- Read the information carefully & Click on "Apply Online".
- Click on “Click here for New Registration”, if you are a new user.
- Complete the Registration & Click on “Submit”.
- After Registration, Log in with Registered Number & Password.
- Fill the all details in the application & upload Photo, Signature.
Apply Offline:
Eligible candidates may apply offline through the application form and send filled in application form with essential to the Chairman, Medical and Health Recruitment Board, Assam, National Health Mission, Saikia Commercial Complex, Christian Basti, Guwahati-781005. The application with all relevant documents should reach on or before 05 Mar 2018.
The name of the post applied for should be clearly written in “bold letters” in the Envelope containing the application form prescribed in Assam Gazette Part-IX.
Self-addressed envelope affixing postal stamp of Rs. 5.00 (rupees five) only should be submitted with the application form.
Applicants must affix 1 (One) copy of the recent photograph on the space provided in the application form firmly with good quality glue/adhesive and also should enclose another copy along with the application. The photograph must be signed prominently.
Candidates who have submitted the offline application are not required to apply online again.
